Nonsteroidal anti-inflammatory drugs (NSAIDs) and paracetamol are commonly used medications in veterinary medicine for managing acute and chronic pain in dogs and cats. By alleviating pain they improve patient’s hospitalization, general well-being and quality of life. NSAIDs inhibit the enzymes responsible for producing prostaglandins which are mediators for inflammation and pain. They possess potent anti-inflammatory and analgesic properties. However, they may cause side effects including, gastrointestinal disturbances, renal, cardiac and hepatotoxicity and coagulation dysfunction. Paracetamol is a centrally acting drugs with effective analgesic and antipyretic effects. It is generally used in dogs when NSAIDS are contraindicated. Nevertheless, its side effects include hepatic and renal toxicity, gastrointestinal disturbances and cannot be administered to cats. Managing pain in patients with diseases such as renal, hepatic, cardiac insufficiency or in patients undergoing gastrointestinal surgery may be challenging. These patients are more susceptible to adverse effects of NSAIDs and paracetamol and there is paucity of safety data and indications for use in these patients despite high pain burden. Factors such as age, breed, concurrent medications, and organ function must be carefully evaluated to ensure the safety and efficacy of NSAIDs and paracetamol.
Clinical implications:
NSAIDs and paracetamol are fundamental in managing pain and inflammation in small animals. While effective when used judiciously, their potential for adverse effects underscores the need for vigilant monitoring and responsible prescribing practices in veterinary medicine. Further research is warranted to assess safety of NSAIDS and paracetamol in patients with co-existing diseases.
