"Lidocaine, Ketamine, Gabapentinoids, Amantadine: from evidence to practice and pack"
Lidocaine, ketamine, gabapentin and amantadine are increasingly used in veterinary medicine to manage pain and are usually considered as adjuvant analgesic drugs.
Lidocaine is a local anesthetic that blocks sodium channels, thereby inhibiting the generation and propagation of action potentials. It can be used not only for local anaesthetic techniques but also systemically to manage acute pain or as part of balanced anaesthesia protocol. Ketamine is a dissociative anesthetic agent that acts primarily by antagonizing N-methyl-D-aspartate (NMDA) receptor therefore modulating central sensitization and exherting antihyperalgesic effects. It can be useful in managing both acute and chronic pain. Gabapentinoids are anticonvulsant and analgesic medication that primarily acts by binding to voltage-gated calcium channels, thereby reducing the release of excitatory neurotransmitters. They are usually used in dogs and cats to manage chronic pain conditions, such as neuropathic and oncological pain and pain caused by osteoarthritis. Amantadine is an NMDA antagonist and is used to treat neuropatic pain and pain caused by osteoarthritis.
Clinical Implications:
Although the exact mechanism underlying analgesic effects of these groups of drugs is not fully understood, clinical studies and case reports demonstated their efficacy as adjunct analgesic/ antihyperalgesic drugs. Further studies are necessary to understand the mechanism of action and pharmacokinetic and pharmacodynamic studies are necessary to identify the correct dose regimen, also in animals with coexisting diseases.
